Polypharmacy Review
General Practitioners (GP’s) in the UK are asked to review the medication of patients prescribed four or more repeat medications at least once every 15 months as part of the their General Medical Services (GMS) contract.
National and regional work groups have recognised variation in the quality and approach to medication review. In NHS Dumfries and Galloway, a practice-based, single educational intervention was developed with the aim of improving GP-led medication review. The unresolved care issues, commonly identified are included in this resource.
